Description des services: Partnership between Mackenzie Health and March of Dimes Canada facilities support to individuals who lives with effects of an acquired brain injury * team includes Program Consultants, Case Managers and Rehabilitation Workers

Service available:
  • case coordination and comprehensive functional assessment to evaluate the effects of the acquired brain injury on the individual and those who support them
  • individualized service/treatment plan, developed with recommendations and community support to promote acquisition of skills and strategies
  • plan which will enable persons with an acquired brain injury to live with maximal fulfillment in a state of dignity within the community

Mackenzie Health, through York-Simcoe Brain Injury Services, also offers Adult Day Programs for people with acquired brain injuries at three locations within York Region

Procédé et formulaires: Self referral, professionals, family members or others with client consent
Admissibilité / population desservie: Require medical documentation of an acquired brain injury and be 16 years of age and older
Acquired brain injury is damage to the brain which occurs after birth as a result of a traumatic or non-traumatic event * Causes include assaults, motor vehicle accident, falls, infections and anoxia * ABI is not related to a congenital disorder, developmental disabilities or progressive diseases such as Alzheimer's or Huntington's
