Bayview Intensive Case Management is designed to provide long-term mental health supports to individuals who are living in the Greater Toronto Area. Intensive Case Management supports the quality of life and overall health needs for individuals by supporting and maintaining their housing and building connections.

Procédé et formulaires:

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
Individuals may also call the program directly for information.
