Description des services:	Offers support to people who have a developmental disability and live on their own * The amount of support provided is dependent upon the stated needs of each person Supports include: money handling and budgeting menu planning and assistance with preparation of meals landlord and tenant negotiation shopping home maintenance assistance with appointments accessing other community resources
Procédé et formulaires:	Contact the Developmental Services Ontario(DSO)- Central East Region to apply for services and support * Client and family/primary caregivers must fill out and sign an Application for Developmental Services and Support (ADSS)form
Admissibilité / population desservie:	Adults with an intellectual disability * All applicants will be assessed by the Developmental Services Ontario (DSO) to determine eligibility with the Ministry of Health and Long-Term Care requirements
