Outpatient Services
Services offered in clinic setting and in the home when patients are no longer physically able to come to the clinic
- complex pain and symptom management
- support with establishing goals of care and advanced care planning
- support with addressing one’s physical, spiritual,
emotional, social and informational needs
- support for patients and families facing difficult end of life decisions such as withdrawal of treatment
- procedures such as paracentesis and blood transfusions
- seamless transition from inpatient care and direct admission wherever possible
