A centralized referral system for musculoskeletal (MSK) conditions * referrals for primary hip and knee arthroplasty are the first type of MSK surgery to be processed under Regional Central Intake Hub for Orthopedics
The centralized intake system for primary hip and knee joint arthroplasty for the region’s five Rapid Access Clinics (RACs) includes: Humber River Hospital, Mackenzie Health, Markham Stouffville Hospital, North York General Hospital and Southlake Regional Health Centre

Information for Primary Care Physicians
All primary care physicians are required to fax completed referrals for primary hip and knee joint arthroplasty to the centralized fax number above * referral form located on each corresponding hospital website
