Case coordination for children with complex social, emotional, developmental, and/or physical needs that require a coordinated plan- 12-18 months of service coordination and case management
- brief support during wait times, new diagnoses, or transitions
- Program involvement is voluntary and confidential

Parents, professionals or agencies with informed consent; primarily agency referral; referrals can be faxed or emailed to dsaccess@yssn.ca

Children and youth birth-19 years who reside in York Region, or up to 21 years if still in school
- currently involved with children's services providers in York Region
- have complex social, emotional, developmental and/or physical needs that require more coordinated service plan
