Description des services: This program provides assessment, consultation, and treatment to older adults with complex medical and psychosocial issues. Services include: - assessment and diagnosis - treatment on a time limited basis, including medication management (pharmacotherapy), psychotherapy, relaxation, and follow up care - case coordination - education and support for clients, families, and professional caregivers


Procédé et formulaires: Referrals from a physician or nurse practitioner for this service can be initiated through Baycrest Central Navigation. The Central Navigation team will assist community agencies, primary care providers, and other healthcare professionals in navigating Ambulatory Services at Baycrest. Eligibility Criteria: - individuals experiencing a mental health issue such as depression, anxiety, behaviour or personality changes, confusion, or significant caregiver stress - able to attend appointments at Baycrest
