A program managed by Humber Hospital that allows patients who no longer require hospital care to return home sooner with the support of community care providers
- offers direct care services based on the assessed requirements for each patient, which might include a personal support worker (PSW), clinical nursing care, and rehabilitation supports such as physiotherapy and occupational therapy
- Care Navigators oversee each patient's transition plan and recovery after discharge from the hospital * community support services may include transportation, meal on wheels, coordination with the patient's family physician, setting up safe and appropriate in-home care * not every patient is an eligible candidate for HRH@home
- patients are on HRH@home up to 16 weeks and transitioned to other community supports for ongoing services, if required
